1. During the past month, did you participate in any physical activities or exercises such as walking for exercise, running, tennis, soccer, volleyball, golf, gardening, bicycling, swimming, wheelchair basketball, weight training, yoga, resistance exercises, or jumping rope?

1.1 In case yes, now thinking about the vigorous physical activities you do in a usual week, do you do vigorous activities for at least 10 min at a time, such as running, aerobics, tennis, soccer, volleyball, wheelchair basketball, heavy yard work, or anything else that causes large increases in breathing or heart rate?
