Training and preparation

The training was well received by healthcare providers, but a written summary was missing. GPs only played a minor role and would like to have been more involved in the project

Appreciation for the intervention

The intervention was seen by NPs as a valuable addition to their nurses’ toolbox. It was deemed too extensive to apply periodically, but rather to be used when deemed necessary for certain patients

Practical barriers to implementation

Healthcare providers did not consider the time invested in this project to be in proportion to the profit in health per patient. Additionally, unreliable pharmacy refill data proved to be an important barrier for implementation in this project

Organizational barriers to implementation

NPs had a hard time addressing medication adherence in patients. There was a lack of organizational support from GPs and a high level op drop out among participating HCPs. Lastly, a good established collaboration between the GPs and CP proved to be an important prerequisite for successful implementation
