  1. Physical activity level is defined as times of physical activity of moderate to vigorous intensity per week. Pain level is defined as 0 (no pain) to 5 (very high pain). Impact of pain on activities of daily living (ADL) is defined as 1 (never) to 5 (very). Perceived fall risk was defined as 1 (much below average) to 5 (much above average). 5 CRT = 5-chair-rise test; 8 LBS = 8-level balance scale
