Table 1 Inclusion and exclusion criteria

From: Survivors of intensive care with type 2 diabetes and the effect of shared care follow-up clinics: study protocol for the SWEET-AS randomised controlled feasibility study

Inclusion criteria

 Established pre-admission diagnosis of type 2 diabetes mellitus

 Discharged from ICU after ≥5 days of ICU care

Exclusion criteria

 Distance from hospital to home >50 km

 Age >85 years

 Major psychiatric illness

 Anticipated to die within 6 months of ICU discharge

 Pregnancy
